This is a summary of pressure equipment unsafe conditions, accidents and fires that have been reported to and investigated by ABSA. This information is provided to make people aware of the general nature of pressure equipment unsafe conditions, accidents and fires. The intent is to heighten the awareness of the general public to the hazards of pressure equipment in an effort to prevent injury.
Note: “Loss of Containment” as used in this report refers to the loss of any amount of process fluid from the pressure equipment.
